Any athlete planning to participate in their sports summer activities must have the proper paperwork on file with the athletic department before they may participate.
Athletes should confirm with their coach that they have the following paperwork on file with the athletic department:
- Current Physical and Parent Permission
- Concussion Awareness Form
- IMPACT test consent form for students who will be in 7th, 9th, and 11th grades, or anyone new to their sport.
